If you would like to be booked in under the NHS, download and complete the “Patient Information Form” and “Medical History” forms and send back to us on the email provided.
You will be contacted once an appointment becomes available, there may be a wait for an appointment due to the high demand for our NHS services.
The practice is located within Basildon Town Square, allowing easy access by car, train or bus. Both the Basildon bus and train stations are within a 5-minute walking distance, and being in the Town Centre there are multiple car parks to choose from.
We are situated within Southgate House on the first floor, there is access via a lift for disabled patients. There are also disabled toilets available.
Monday to Friday 9am to 5pm, closing for lunch between 1:00 and 1:30.
Some Saturdays are available for Private appointments.
